Yes, a custom muffler shop will be able to bend you one as long as there is no cat on the truck. Im not sure where to get just the down pipe unless you can find a wholes used system for cheap, and just use some parts.
tell im what you want and he can get it to you. He offers a 5" system with a 4" downpipe in aluminized for like $350. Probably even less if want it to exit bfore the rear wheels.
I did it to my truck. I used a 5" turn out stack and brought it out at an angle. I actually put the turn out part on my regular exhaust and butt welded it to give it the angle and the tip is about 4" from the rear tire. It took minor cutting) then I mitre cut it off level with my tire and running boards then welded a hanger from the stack to the frame to hold it in place.
PS. There is really no bent. You just need the Down pipe and the connector for it on the other side. After that its straight to about a foot before the bed. I can get 1 foot of 5" for $6.02 Prob cheaper if your getting a whole or half stick.
I got a kit similar to this, from Dallas at warpspeedperformance, 4 but I also have the advantage that warp speed is less than a mile from my house. The owner and I put on the 4" downpipe and sections back in the similar fashion as you are wanting and he charged me $165, I'm sure if you call him he will quote you what you want.

Did the stack kit come with a bend or with flex pipe? I have heard not to use flex pipe from a couple people, but I have never asked a professional.
My origional dual stack kit came with a flex pipe but i never used it. It fit without. Then i sold my stacks cause i hated them and had my exhaust lopped off right at the bed, then a buddy gave me 1 stack and thats what i did with it. Ill try to get a pic if the rain ever go's AWAY!!!

My boss has a setup on his F250 that seems like that, It looks pretty simple, it sounds really nice outside, and it really doesn't make a whole lot of difference inside the cab.
The problem with where I live in Ohio is that there are limited parts dealers for anything bigger than an F150, and even then we tend to have trouble. I had to search for a long time to find a place that stocked 4" pipe regularly.
You can goto Napa for 4" they sell it by the foot. Im the next state over (PA) and I have heard of a few good diesel places in Ohio i just cant think of them off the top of my head.
