My truck('91 diesel, e40d) has 165k miles in it. I don't know if the oil in the transmission has ever been changed because I just bought it. Does any one have step by step instructions on how to change it? Also, how much oil does it take, and should I change the filter?

The Haynes manual has the whole procedure and this site probably does, too. Basically, get a BIG drain pan & loosen the pan bolts so it tilts slowly and dumps the fluid out one end until it's empty enough to remove. Clean all the sediment out of the pan & off the magnet, and now's the perfect time to add a $4 drain plug kit (MotorMite HELP! at most parts stores). ALWAYS replace the filter & pan gasket (they come together), and plan on about 7qt of Dexron/Mercon even if you don't drain the torque converter.
If you really think it hasn't been changed within the last 40Kmi, plan on doing this again in about 1000mi to clear out all the sediment from inside the transmission. If you don't, you could be rebuilding/replacing it soon.
I believe Mark Kovalsky did an excellent write-up on this site on how to flush your transmission. If you decide to just drain the fluid, make sure you drain the pan and the converter, and it'll hold near 15 quarts.
