When I bought my 2001 F250 it had a 5th wheel in the box. The previous owner left the plates attatched to the bed. My question is does anyone know if there is an adapter or something so I can put a goose neck ball to the plates?

It probably depends on what you currently have. There is a fairly common set of "rails" that go horizontally across the bed in front of and behind the wheel wells, attached through the bed to heavy angle brackets to the frame.
If this is what you have, then there is probably a gooseneck plate that can go on it.
For example, this is a gooseneck plate that goes on a number of fairly common fifth wheel rails.

Here are a couple pics of under the truck should I take the brackets,plates off and check everything then replace the bolts too? Or just bite the bullet and replace all?? I have seen worse considering I am in Illinois and the truck is a 2001.

At least take them off , sand blast brackets, check for cracks, paint , reistall with new bolts and you should be ok.
My neighbors rail sat up is more rusted then that. If its only on the surface its good to go. I would be more unsure about only the one bolt holding the bracket to the frame. I would add another bolt on all 4 frame brackets. IMO
The installer was really lazy on that job. The install kit with those bed rails come with 2 bolts per bracket. They are in fact a fairly common rail that fit quite a few hitches as was mentioned. The latest rails also have another bolt in the center of the rails, one side or the other depending on where it would fit without a cross brace in the way.
Having one bolt per bracket through the frame is not safe. On hard acceleration / deceleration, the bracket can turn around the one bolt, allowing the hitch to move and destroying your bed. Take it to a reputable hitch installer to have it evaluated.

I wanted to update you guys and say thank you very much. I added the second bolt to all the brackets and took a close look at the rails and brackets themselves. I can see why the installer only used 1 bolt per bracket it was kinda tough drilling through that frame. However thats no excuse I feel it should be done right the first time. I got it done though and appreciate you guys letting me know about this because I wanna be safe for sure.

For towing a 5th wheel I would get a B&W Hide a ball set up. They are 500 bucks, bolt right in under the bed and the best part is you can use the bed for what ever you want cuse there no rails!
