zf 5 speed problems

i have a 94.5 f350 single cab 4x4 5 speed. the problem started when i tried to start my truck and the clutch went straight to the floor.so i then dropped the tranny and noticed that the lever that goes from the slave cylinder to the throw out bearing had come off of its fulcrum and the throw out bearing was completely trashed. i then got a new throw out bearing and put everything back together. went on a test drive around the block(had to start out in 2nd gear because it wouldnt go into 1st not to mention that reverse would grind a little when i put it in) and my clutch pedal went straight to the floor again. the slave cylinder fell off and the lever came out of place again. if anyone has any advice it would be much appreciated

theres a few things you need to look at...

#1 how is your clutch rod bushing? have you done the permanent fix?

#2 how is your pedal box bushings?

#3 is your pedal box cracked/busted?

Sounds like the clip holding the slave on is broken to me.
