Yes tried the start and restart test. The fuel pressure is ok. It did happen slowly, I thinking MASS air flow now. It seems to change with the weather.

I would start at the throttle body and idle control valve cleaning at this point. Not sure, but the mass air may be 'ignored' by the system during cranking; pretty sure it was on older engines. If it were bad you'd probably get bad running and a code, check engine light would be on. If it's just dirty you can get a MAF spray cleaner, they work well. You could try disconnecting it and see if it changes your start-up, tho it won't be absolutely conclusive. Also could be MAP sensor or hose leaking. If fuel is ruled out and if when it starts it comes on somewhat slowly, I'd say air flow. If it fires and immediately jumps to the proper RPM for the given engine temperature then it's probably something in the ignition control and I'd start at crank position sensor.

The crank sensor has been replaced. As for the IAC, giving gas while cranking would bypass a bad IAC.

It will bypass it to the extent of allowing airflow, but will send other mixed signals due to the TPS that may actually cause a hard start.
