So I had a rough idle problem, that seem to get worse when the engine warmed up, or when the outside air was hot. The roughness would sometimes linger at low RPM operation, but would clear up if the vehicle kept moving.
I had been fighting this for a while. I replaced the ignition system basically (improvement, but the problem was still there). It had developed a couple vacuum leaks, fixed those, improvement again, but problem was still there.
Finally, let some pros have at it, they found the fuel pressure would drop after the engine was turned off. Their first suspicion was that the fuel pump was bad, at least the check valve in the pump. But then I informed them that I had recently had the pump replaced at the dealer.
So the isolated the pump from the system and pressurized it again. Fuel pressure still drops.
The isolated the leak to the #3 injector. They removed all the injectors, told me that all the others were in good order. So I had the injector rebuilt. Guess what, that fixed the problem. The van now idles smoothly.

I dont know if it helps but I run Lucas fuel cleaner ( Upper Cyl lube) in the gas tank at every oil change. Engine still runs faultless after 300,000Kms. level drops 1/8" on the dipstick between oil changes.
Well, I did not see the old injector, but the shop tells me it was leaking externally as well, which leaks me to believe it had cracked. I ran cleaners though mine often anyway, but I'm not sure thats even an issue, most fuels today run pretty clean anyway.
For the record, mine has a lot more miles too. Yours would equate to roughly 186,000 miles, mine has 240,000. Mine looses a little oil through a leaking oil filter bracket, but even so I only add a quart every 5,000 miles. I need to get that fixed, the problem is the gasket that goes between the filter adapter and the block.
