My first thought is a bad fuel injector.

Check the electrical connections at the injectors and also the vacuum lines, especially the elbows on the PCV hoses.

Replace the fuel filter lately, or is it original?

Do a fuel pressure test to make sure you're up around 45 PSI at idle. If it's below 40 PSI you have a weak fuel pump.

50K? Ouch, I'd start with replacing that to see if the problem clears up.

Have you relieved the pressure on the fuel system before trying to disconnect the lines?

If you have a schrader valve on the fuel rail, you can bleed off the pressure there like letting air out of a tire. The down side of this is you have gas running out. The safer way is to pull the fuel pump fuse and try to start it. It should fire up and die right away. This should relieve the pressure in the line.
