Rear door trim

Rear door trim
I'm looking at an F150 to buy. I've found a 2004 F150 Lariat that I like. It's got a couple things that concern me a bit.
1. On the passenger side, the rear door trim has been damaged. The guy who is selling it bent it back and ran what looks like a drywall screw into it to secured it. This is something I'd like to replace, but I'm not sure how much it would cost, or what part to even order. (is that trim separate, or do I need to order the whole door?) I've tried to attach a picture showing the area I'm talking about.

2. The truck has 91,000 miles. Is this too high? I've read about some of the problems people have with this engine, seems like most of them hit pretty early if they're going to be a problem.
3. The guy mentioned, and I noticed a slightly rough idle. I read a little about some things that can cause this. It wasn't super noticeable. He said it would go away if you tapped the gas.
Other than that, the truck ran great. Is there anything there that jumps out as a point of concern? I'd like to try to get another 100,000 miles out of the truck.

I dont know about #1, but as far as #2, I bought my 04 with 95k on it and I never heard it run until after it left the bodyshop. I dont know about others, but I have had no problems with mine. I will say that maintainence plays a huge role. I wasnt worried about mine once I saw how clean the truck was.
Come to think of it, I think that rear door trim piece is called an applique?
