94 Explorer automatic not upshifting

My son's 94 Explorer, with an automatic transmission, is experiencing a peculiar shifting problem. The truck accelerates smoothly but won't change gears. The experience is akin to driving a manual and being stuck in 2nd gear. When you take your foot off the gas, the RPMs drop as you might expect. Applying the gas again doesn't change the situation.

I noticed that I can get it do upshift if I put it into Neutral and then back to Drive.

Ideas appreciated.

My first thought might be a vacuum issue (vacuum line to the transmission disconnected or plugged). 2nd thought would be some problem with the vacuum modulator or governor.
