Well it's happened again oil in the water. Just fixed this two weeks ago and three months before that. What's going on with this thing? Am I missing something ? Changed out oil cooler both times, removed front cover and inspected it and now I don't know what to do with this -----. My funds won't let me send to dealership. Please help I'm at the end of my rope with this engine....
Just Curious: How healthy is the cooling system on your truck? Was it healthy when You bought it? Just throwing this out there , could there be a hole in a liner or could one of the heads be warped slightly allowing the mix. I once had a 425 3406C Cat that wore a hole in a liner and it was doing the same thing. Wish I could be more help.

I 've thought about it being a hole in block but this is the third time, wouldn't a hole in the block show up as soon as I changed oil cooler, I mean it's been perfect since I changed it, now it's doing same thing. I don't know I'm stumped why this keeps happening.

Not a diesel mechanic (jet aircraft though), buy a exhaust gas tester and test for exhaust present in your coolant system. Then go from there, it would not take much of a defect with diesel engines given the compression to move oil or gases into coolant system, cheap elimination process as well.
rodolan, I,m sorry I can't be of more help to You. Just threw that info out to get you thinking about every possible ption> I know of people who just start throwing parts and money at equipment w/out looking outside of the box.I will check w/My Garage on Monday AM just to se what they might think. take care!
Have you also replaced (or better yet removed) the EGR cooler? If the first bad oil cooler plugged your EGR Cooler, you could have limited or no flow through the Oil Cooler causing the failure.

How about the screen in or under (not sure where it is exactly, haven't been in mine yet)the oil cooler, I think it is only like 15.00 bucks. Just throughing this out. Let us know what you find.

Good luck!
Yes I took out the screen and cleaned it when I replaced oil cooler. Thanks for all the questions, maybe I missed something but after doing it twice already, I don't know.
Don't know about this one but did you do a complete flush of the coolant system and are you using a coolant bypass filter to help rid yourself of any additional sand?

I flushed the system out with cascade and no I don't have a coolant by-pass filter.
