nobody did really...
the long bed was simply a regular bed with an extension... With heavy use, you could sometimes find they would start to separate.

Bought a 2006 dually 6.0 short box a couple of months ago. I have a fifth wheel hitch and tow a 14,300lbs. camper with it no problem.
The hitch actually moves back progressively as I turn allowing the back of the crew cab and the front of the camper more separation. I believe it moves 12" or so at maximum turn.
