I can save you the trouble of having to figure it out on your own. I've posted a pretty much step-by-step of how I remade my supercab headliner in my gallery. I'll post the link below.

It's made of an ABS plastic backing and has 1/4" foam and black vinyl as upholstery. Should basically last as long as the truck as long as I take care of the vinyl. If you have questions, let me know.

I had the same problem didn't find it new anywhere, I think because of the size, finally gave up and find one in a junkyard just painted the same color as the truck.
