I dunno, my truck does it probably every 10th start, done it for years. New starters and ring gear, no go. New ring gear did help it for a few months, but I think the flex plate is bent slightly.
I've heard that 7.3's are notorious for having the starter bolts come loose on a regular basis. Mine did, and so slowly that I never really noticed it was loose, until it suddenly wouldn't start at the tire shop when I went to pick it up. I borrowed a creeper and slid under there and the starter was about to fall out.
After I tightened it, it sounded like a brand-new truck. So apparently it had been loose for quite some time.

Nah, I don't recall, but I would put my money on 9/16". Just take your whole socket set down there with you.
BTW I am pretty sure there is at least one thread on this subject, and it probably contains that info. I think they might have talked about using Loctite or something on the bolts also.
