Engine surging at low rpms/speed

I have an '04, F150 Lariat, 4x4, 5.4L V8, ~100K miles, original plugs. Engine surges just a bit at low rpms (1000 to 2000 rpms) and low speeds (25 - 40 mph). Higher speeds - not much of an issue. But, if I get it to say 60 mph, coast a bit then gently accelerate, it runs rough till I slightly punch it to change gears. Fuel mileage is about 16 - 18 on back roads (~45 - 60 mph usually, no frequent stopping). I know I need to change plugs and fuel filter, what else should I do? Thanks for help.
