A cat-back exhaust is when you replace the exhaust pipe from the catalytic convertor (cat) "back" to the end of the tailpipe. Usually this is done with a larger diameter pipe to lower backpressure & give you a little more horsepower. Also, often the muffler is either replaced with a low-resistance one, or left off altogether, either way making the truck (or car) sound louder.

I did this to my 94 F-150. The pipe diameter was replaced with three inch and a flowmaster muffler was installed. There will be a sigificant increase in your power. If you keep your foot in the kitchen your gas mileage will drop. If you drive normal you'll be ok! By the way this was from single to dual exhaust.
