Control Security Module (CSM) - driver door lock switch

Having issues with driver door lock switch not working unless key in ignition and "on" along with inability to arm security system. No doors are ajar and I believe door switches are all fine (did a check) and issue is with CSM.
Anyone know if one can be purchased and installed without going to a dealer? Do they have to be programmed?
What's an approx. cost? - I can't find anywhere online.
What would a dealer charge to remove and replace the CSM?
