Help on running issue.

OK I have been reading previous posts and searching the forum to try and come up with an answer to my problem. My 03 will idle irradically within the first 5-10 min of cold start. I will crank it and it sounds like it almost has a hard time starting, it will start and grunt to come up to idle. I will drive down the road to the redlight by my house (about 2 miles with 1 stop sign in between) and stop. The truck will surge up 300-400 rpm from idle then down to 450-500 rpm. When I hit the gas the rpms jump up and down real fast as if I am pumping the throttle then it goes. It will only do this once at the red light and then no problem. (maybe it is a diesel grimlin at this light I do not know) The idle surge will ocassionaly happen throughout driving but not often. Also the truck on cold startup will sometimes do that thing I described as pumping the throttle quickly when I put it in gear for the first time. As long as I let off the throttle then reapply it will quit. I have been thinking ICP sensor but all the posts say that a code and CEL was thrown and I do not have either. HPOP (please no but maybe) I am running out of ideas please help. Thanks.
