Unhappy Hard Starting Cold

My 2000 F-350 (368000m) has developed a cold blooded attitude this fall. It just won't start without being plugged in below about 20 degrees F. I cycle the glow plugs and all it does is crank, lots of black smoke and once in awhile I'll get lucky and it will start with lots of cranking. If I plug in the block heater (4-6 hours ) it takes right off. Also - when started cold it sometimes misses, but usually not when plugged in. Replaced the air preheat solenoid - seemed to help but not to cure. Anyone have any suggestions? I'm thinking maybe tired glow plugs.....

it may be glow plugs, or it may be the glow plug relay.
