Update: I replaced the IAC today, wasn't to bad of a job, other than it was on the backside of the throttle body and we just had the intake manifold off last week to do the coolant return tube that ran under the manifold from the rear of the motor to the water pump.
I guess I'll find out when the temps drop below 30 again, it only happened three days a couple of weeks ago when we had that cold snap and snow in October.
How importent is it to change the gasket when cleaning the IAC? I cleaned mine 2 weeks ago after a thread came up. When I took mine off I didn't find what I thought was a gasket, but I could feel some rubber seal on the throttle body side. So I just cleaned it and put it back on. Now seeing SDDESIGNS picture I see what the gasket looks like. Since cleaning the IAC my truck does idle more smoothly. I have also noticed my idle is higher in RPM.
Should I take my IAC off and replace the seal? or should I be fine?

I did find the gasket pretty well seated in the throttle body, I used a small hook tool to get it out. It was a little flattened, but I always play it safe when ever changing something to replace the gaskets too!
None of those gaskets even look close to what the gasket looks like, plus they look flat and the replacement gasket fits into a recess on the throttle body.
Well it seems to have fixed the cold weather stumbling, we had a successful start without any issues at 27°F this morning! From this point on I guess no news will be good news!

Could this starting issue happen when it's not too cold out. Say mid-day in the 50s after it had been started in the AM?
Yep. My 97 f150 did it at that temp. Once I cleaned out the IAC with intake cleaner spray the problem was gone. The IAC was way easier to get to on the 4.6.
