I've had my Motorola (Nextel) i550 since first introduced. Dropped three times from my Harley and twice from the roof. Still Ticking!
I solved the drop and lose problem by purchasing a case with the dring style clip that cannot be removed except by turning it upside down. Works great for me.

I think I may have narrowed it down to the Samsung SPH-A500 or the LG 5350. I love those flip phones!!
I should just tell sprint to get a tower out by my summer place and I wouldn't have a problem!!!
Qualcomm GSP1600, Doesn't quite meet your specs. but has served me very well. Has taken a beating and so far has not stopped ticking. Tri-mode and it really DOES work out in the "sticks".

I never did like nokia. Had too many problems with reception. I love my Motorola I-1000 plus, well I loved it but I lost it on the side of the road somewhere. Not many problems and that thing has taken a beatin also. Dropped 2 stories, ran over, dropped on concrete, thrown around in my truck, ect. Worked good until I lost it and not too bad on the battery, I could get 2 1/2 days outta a batt as long as I turned it off overnight, and 2 days straight.
