How is it idling? Did you get that piece of crap CPS out of there? Pull the plug on the ICP and see if makes it idle better. Look under the plug for signs of oil. If oil in under the plug, it usually means a failed ICP. Check the IPR as well. The little tin nut that holds the relay together will get loose or come off causing it to run bad.
Two of your codes point toward you EBPV. Sounds like it may be bad or unplugged.
The 1013 and 1220, no idea.

I would agree with this. Pull the chip out and eliminate that as a possibility. Some of those codes don't even show up in the manual. A loose or faulty chip that throws phantom codes can cause the PCM to not operate the engine correctly.

Yes that's always a possibility with a sensor. Since the ICP sensor is reading pressure and providing data that helps the PCM control the IPR, it might act better under certain pressures than others. If the idle is rough at idle, try starting the truck and letting it idle with the ICP unplugged. If the idle smooths out with the sensor unplugged, the ICP is bad. As mentioned above, oil inside the ICP connector is another sign of a bad sensor.

The unplugging trick doesn't work well at higher RPM's since the computer is guessing what readings the ICP should be providing.
