I bought the truck used back in 03 with 2500 miles on her. I was told then that it was the GM's personal ride. He used it to pull his wave runners. The thing that I find funny is after many time under the truck (on my back ) I only notice it now LOL. Damn am I observant.
When your truck is running, does the fan spin?? Seems like it would gather up quite a bit of road debris and damage it. If it turns out to be something that you do need, you might want to make sure it is still working?!?!

To me it doesnt make much sense where it is placed, if it was for the power steering , or a tranny cooler. I would think more air would get to it from the front of the truck.

Yeah, me neither, which may be why it has the fan. But, if the fan is rusted up and no longer works, then whatever it is, is doing nothing!!
Well now, that's the darnedest thing I've seen in a while. Maybe a earlier version of a seat heater? Or maybe some sort of air ride susension? Maybe even used as a vertical take off tool. As far back as it is mounted, about has to be a tranny cooler, a cheap one to say the least.
