It's just the stickers.

the ones without the 'off road' can only drive on the road.

depending on year it could be skid plates or an interior trim package(usually the newer 150s)

For the '99+ superdutys it was a gastank skid plate, red colored boots on the standard shocks and the gaudy stickers.

'04+ F-150s is a separate trim package level

'97-'03 F-150s got a 17inch rim, fog lights, sticker, and skid plates.

For our years of trucks just a sticker, maybe different colored shocks.

My 94 F-250 is the "off road". This had no effect on me buyin it. However, it has a skid plates covering the transfer case, and both gas tanks. Also, mine has 1 ton leaves in the rear, but it still has 3/4 ton axles. Others that I have seen (just 4x4) don't have the skid plates and a lighter rear suspension. And it can go off road.
