If you want to re-register an old car, a simple receipt of purchase will usually do it......after a MASSIVE Roadworthiness inspection by a licensed mechanic....
.....unless you do mods,.....then you need a certificate of compliance issued by a government authourised ENGINEER.
so yeah......none of those 'title' issues,....just getting the automotive equivalent of a cavity-search at registration time
Fast forwards to today. Most vehicles today are financed, and the banks demanded they be able to hold the "deed" to any vehicle until the lein is satisfied to prevent a major court fight over ownership should they need to repo their "collateral". So the states were pressured by the feds to institute a system of "titling" vehicles. The titleholder is the legal owner, and only the titleholder can transfer ownership. This is a separate document to the registration, which is only a receipt for license payment.
All states have a process to gain title to a vehicle where no title currently exists due to age or lost paperwork, but it is often an involved and convoluted process that in some states results in the vehicle being titled as a current year vehicle rather than year of original manufacture. This dating can heavily impact the taxes, registration fees and safety/emissions regulations it must pass to be licensed. A project vehicle that has a "clean/clear" title is a major plus.
