The smoking is probably due to a faulty glow plug(s) or the GPR. Check the GPR first by jumping the two large posts with a screw driver before you crank it up on a cold start (it's located to the right of the alternator). It should spark. Hold it there for about 60 seconds or so and see if this makes a difference in the smoking.
However, it sounds like you have a bad glow plug or two. The easiet way to do this is check the resistance on the plugs. There are 4 valve cover harness, 2 on each valvecover. Remove the pigtail and test the two outside pins. (2 pins per plug, 4 total pins per valvecover). Resistance should be pretty low. Around .3-9 I think. If it reads infinity they are bad.
Also, need a little more information about the truck. Auto/manual tranny? Any mods, etc.
I haven't the slightest clue about the E4od. However, there a ton of people on here that no them like the back of their hand. I'm sure they will chime in shortly.
