Highest mileage Modular I know of is in an E-Series van, a '97 5.4L, with over 1.2 million miles without a rebuild.
Most Modulars are taken out, barring poor maintenance, by either a bad fuel injector, a bad wrist pin bushing or a timing chain/guide failure. If the wrist pin bushings are in spec from the get go (by far most are), and the fuel system well maintained, they go well over 300K on average. Timing chain guides begin to get "iffy" above 300K.
If you've got a good running, high mileage Modular, replace the timing chains/guides/tensioners around 250-350K, you'll likely have an engine that goes beyond 500K. They just don't seem to wear out within a reasonable time, even the really high mileage Mods show little cylinder wall or bearing wear if decently maintained.
