I believe its' when carbon builds up from lack of true heavy use. Every once and a while you have to really punch it and let it get up there boost wise to burn off the buildup that gunks these turbos up.
i have been involved with turbo diesels for for nearly 50 years and have seen a whole mess of them, never have i seen one "Carboned" up.not even on an engine that was burning oil like crazy from cracked rings. they will get a little black or gray dust like deposit on them,,thats normal .

The 6.0 and 6.4 Powerstroke diesel use EGR so exhaust gas is introduced into the intake prior to the turbo so carbon can build up. Also, these diesels use VVG turbos that have moveable vanes on the compressor wheel that need to be exercised to keep them from sticking. The days of simple turbo's are just about over.
Take it to your dealer, they will try to clean the turbo or replace it with a new one. This is a somewhat common problem on the 6.0L trucks, and is normally seen in trucks that don't get driven often, or get driven on a lot of short trips.
Do you have an aftermarket air intake? Some of those have also been accused of increasing the problem.
The turbo is covered under the 5 year 100,000 mile emissions warranty with no deductible, don't let the dealer try to convince you otherwise.
