Fuel Injector Pulse Width
I just bought a 94 Explorer XLT 4.0L and the only problem I can find is a continuous code being thrown that says the Fuel Injector Pulse width is either hi or low. I'm wondering if the code (186 I believe) is an indication of the input to the injectors or output. In otherwords is the most likely suspect the injectors themselves or what ever tells them to fire. The engine seems to run reasonable well regardless of the condition.

CM 186 doesn't point to a problem in the fuel injectors. It points to a problem in the sensors used to determine fuel mixture: principally MAF and O2 sensors.
Does it pass the KOER test? Have you cleared CM to see if this code keeps coming back or not?

Yes, it passes the KOEO and KOER. I cleared the codes from storage and it always comes back about 5 minutes into a drive (generally under heavy load conditions). Tested and cleared about 3 times last weekend. That's good to know that it might clear up with a MAF cleaning or O2 sensor change.
