Had a similar problem on my '79. Ended up being a small hairline crack in the distributer cap that allowed moisture into it. I'm not sure if your '89 has one or not, but I'd start there....
Well, I had a starting problem with my 84 that had me scared to take the truck anywhere for fear it would never start again. Every once in a while it just would NOT start. I mean not start to the point of killing the battery trying. The mechanic would come to my driveway, look at it sideways without even touching it, and it would run. Turned out the fuel pump was dying and every once in a while it just decided to quit ... and then work perfectly well for a while ... and die again. Never had another problem with it starting short of occasional flooding after that ... until I hit a flippin' deer with the thing.
When its cold , Get a reading on the fuel psi , specifically when you turn the key to run , If the psi raises slowly & drops off quickly First replace the fuel filter if thats not been done in the last 30K miles . If replacing the fuel filter doesnt change things & the fuel pump psi builds slowly & drops off quickly ...Id suspect the pump , Ive been thru 2 in 180K miles for the symptoms you describe . Normally 2 cranks & the engine lights up . How many miles on your truck ?
I had a cold starting problem with my 1987 4.9L. I did a complete tuneup, with plugs, wires, distributor cap, rotor, coil and I still had a cold start problem. The last thing I did that fixed the problem was to change the intake by-pass valve (about $75. from the Ford dealer) now it fires right off every time no matter how cold.
Good Luck
If the fuel pressure checks out then I would go to the idle air by-pass valve if it don't work it will not idle right The way the idle is controlled on these efi motors is by how much air is allowed to by-pass the throttle plates. You can take them off and clean them.
I take the solenoid off and then give them a good bath in carb cleaner.
