how hard will it be to install them with the fenders on
Are you talking about the inner fenders? IF so, you couldn't install them until you did some cutting, but once you do some fitting and tracing you could cut out what's needed and put them on.
Other than that I haven't actually installed fender well exit headers, so I'm not sure if it'd be easier or not.
I'm sure it can be done either way, but I don't know anyone that pulled the fenders spesifically to install headers.
