Sounds a lot like an electrical proplem.
Could be the Vss, but could also be the FIPL sensor (Fuel Injector Pump Lever sensor).
You can check the FIPL sensor with a voltmeter with the ignition on, and the engine off.
Check for smooth voltage climb and drop to make sure that you don't have any dead spots in the throttle range.
The electrical connector on the passenger side of the tranny can also cause intermitant issues if corrosion gets in.

do your tach and speedo work good? i hear they make big diffrences in wether or not your trans dies ...uh yea thats all i got
Does your truck have cruise control? If you do, and its having trouble holding a consistent speed, then that would also point to a VSS (located at the transmission end of the speedo cable).
The speedometer is not electonic untill 1992, so it will not be affected by electrical issues, but a wobble in the speedo could indicate a problem at the drive gear itself, and that could throw the tranny off since it needs a consistent signal to work properly.
you need to check the pcm for codes , the 90 model did not have a malfunction light feature so there will be no warning lights , you can waste a lot of time and money by trial and error, checking voltage at the fipl wont find a concern there if the sensor has noisy operation your basic choices are the fipl ,the speed sensor , the tach sensor , or the manual lever position sensor can all cause your concern
