Did you put a new pressure plate on? Did you resurface the flywheel?

Also, is it a wet clutch?

If it is indeed a wet clutch, I could see not bleeding it properly causing problems when the fluid heats up

The flywheel should be resurfaced whenever you do a clutch and there should always be a new pressure plate

I would try and fix that leak first and see where that puts you. It is probably the slave cylinder. It shouldn't make a huge difference, but I have found that if you get the obvious stuff taken care of first, the small problems go away.
