I have a '00 F450, 179K miles. It is a service truck, so it has a 2-way radio, and some flashlight chargers and such on it, so I leave it plugged in. I have an on board battery charger, as well as a block heater, and have them wired in such a manner as each one has it's own breaker, but I only plug one thing in.
Anyway, after I got my last electric bill, and fell on the floor, I remembered I had not shut the breaker on the truck off for the block heater. I know it does not draw that much, but every little bit helps.
The next morning, I go out to start the truck (keep in mind it is summer time - 85* at 8:00AM) and when I light her off, I get the romp-romp, like I am starting it in the winter. It smooths out fairly quickly, though.
The next morning, same temp outside, but with block heater on overnight, starts like a champ.
Go see Kris, he is a neighbor of yours. But seriously, I think my romp romp is oil related, at least on initial startup. In the stock setting, turn the key and crank immediately, romp-romp for about 3-4 romps. In the 80econ or elevated oil pressure to injectors, starts up like a champ. I am still looking into this. I am wondering if it is IPR, ICP, or even the HPOP. Once lit or in higher tune, runs like a champ.
