Well i have a 2004 F150 STX, so far its completly stock. at about 53,000 miles i noticed that when i started the engine sometimes it would be SO loud, i would shut it off and start it up again and sometimes it fixed the problem. if shutting it off didnt work i would drive it until the engine was warmed up and then shut it off then start it back up and it would usually be quiter. I love my truck so much and im just really concerned about it, because it took alot of work to get it. On another forum ive heard of peoples cam phasers getting louder and having to get them replaced, im no mechanic so i dont know if that could be it. ive also noticed that if it has more than a 1/4 tank of gas it usually doesnt start up loud. And since the starting issue ive noticed that when i accelrate it gets REALLY loud. Im sure its nothing major, im just paranoid about my baby.LOL. Does anybody know what could be wrong with it??? Oh yeah, the engine vibrates badly, you can feel it in the steering wheel and i can also feel it with my head resting on the doors arm rest. By the way its the 4.6 V8

before the engine was really quite, it seems like hi rpm's but since i dont have a tach im not certain. i could hear the engine from easily 50 feet away at idle.
See if the fan spins fairly freely when the engine is cold. If the fan clutch is starting to get sticky/stuck, you will probably notice the fan roar being a lot louder than you're used to at startup.
I don't know what the exact specs are for a good clutch, but if the fan blade wants to flex at all when trying to push it by hand on the tip of the blade, I'd say the clutch is going bad.
It seems too soon to happen on an '04, but it's worth checking.
The 4.6 usually has (at least mine) a nice start-up "roar" on the morning crank. Its been that way since it was drove off of the lot (new & 50K+miles ago).
Fwiw.... all of my past Ford p/u have had a similar roar...
....As for the shaking, does this occur after the engine has warmed up and is at a standing idle?

Im still trying to figure out the loudness "sound". Is it anything Like the "start-up roar"...but just lasting the entire run time? Have you had any recent exhaust or fuel rail work done? How is the exh sys.overall? Can you localize where on the truck/motor the "loudness" is coming from?
Maybe time to let the engine mgmt controls to relearn.....
Possibly something has or is in the midst of failing (or out of calib)... engine maybe trying to compensate.
...Strange gas?
Any Check engine lights?
Jus' tossin' ideas out...
