after it warms up...do the two heater hoses feel hot?
if tehy do, then it could be the heater/air control cable...remove glove box and check the movement..you can remove the cables there; center it and reattach the cable end.
and, if the hoses are not hot; esp the return hose..could be:
blocked inlet hose
water pump
I can't recall if the thermostat can poss. be installed upside down, so...the thermostat is in right?...and engine would not heat up well at all

My 1994 Bronco (302) has the same problem. If the outside temp is above say 10F then it will eventually get warm. If it is colder than -10F it will never get warm enough to not see your breath. I was suspecting the fresh air vent is not closing enough. I also never have frosted window problems.
But I have no idea where under/behind the dash to look.
