PVC valve

There is a box shaped device on the left valve cover with a hose to the air intake. What is in the box? Is there something I need to change periodically? When I check the oil on a cold engine it shows 2 quarts low. When I take off the oil filler cap it turns off hard and sometimes I need a pliers to get it lose. Once the cap is off the oil level shows full. It acts like there is a vacuum or something holding it on. Could some smog device be causing this vacuum?

the box you see is the crank case breather vent (CCV) it allows pressure from the engine to be blown into the intake. as far as your oil fill cap there should very be a vacum. these engines have enough blow by (normal) to never allow any vacum. also when you check you oil is the truck running or not? other than the time from when it is shut down there should be no change in the oil level.

it is possible that the CCV is plugged and putting pressure on the cap. you can remove that housing. it has two philips head screw holding is down and use mineral spirits, gas, or brake cleaner to flush it out. it has very course steel wool type stuff in it to seperate the oil from the vapor. jsut make sure all the cleaner is out of it before you reinstall or your engine may "run away" ie over rev from the extra fuel.
