I would guess it is the egr valve that is not working properly maybe plugged. I would start there and see where that gets you. This might take care of all the codes a stuck egr valve can trip the lean codes.
Usually this is caused by a restricted exhaust (usually the catalytic converters)... Those could have overheated due to your lean running condition caused by your vacuum leak.
You should probably take it to a professional mechanic on this one.
