The major downside to using the EBV as a brake is if it ever malfunctions closed and you don't notice it it will melt the turbo to slag. I've only seen it happen once, and that person was using an aftermarket brake.
I use mine as an exhaust brake, and haven't had any problems. I think i would definately know if it was stuck closed... sounding like a jet and huge lack of power... would def. make me fix it!!
I have had mine hooked up for almost 2 years, no problems. I have noticed it is getting weaker as time goes on. Not sure why but it still helps hold back a trailer

Check your up-pipes to the turbo. If they are leaking the pressure will vent and will give the sensation of the exhaust brake weakening.
(Personal experience here. )
I know when I had one bolt missing from my turbo exhaust housing I couldnt feel a difference with the ebpv on or off. Up pipes leaking..... that doesnt sound like fun. I will see what I can find. How big of a pain is it to fix?
I fixed mine while the transmission was out for repair. That made it a cakewalk. It's a medium PITA if you have to do it with the transmission in the truck.
