A young guy I work with is in university to be an electrician and in the three months of classes they kept stressing the importance of safety in the labs and work sites and sure enough, there are no incidents. On one day where there was an optional study class, about nine students are in the class room and one kid in the back is goofing around. Leaning his chair way back and holding on to some sort of copper air or gas pipe, while in his other hand he has a metal pen and while his hand is wandering aimlessly through the air as he rocks his chair back and forth, he manages to stick the pen in an electrical outlet and electrocute himself. After falling off his chair and blazing curses, he gets up and explains, amongst the laughter what he has done and they carry on with a problem solving exercise that figures out how much power would have gone through him... complete with drawings of a dude rocking on a chair and a pen in his hand.
I was literally ROFL when he was telling me that one.
This happened to my boss today, he was lifting one end of a large compressor, with a 2x4, to help get moved. the compressor was outside and some ice had developed. so anyway, he is lifting with all his might and both feet go out from under him and he goes full tilt into the motor, with his forehead. He said he was waiting for everything to go black, as people were telling him that his forehead was bleeding, and that he didn't know he had that strong of a head.
