On my wife's 91 extended, when driving, the sliding door "clunks" on rough roads, etc..., like it's not fully closed. However, when you get out, not only is the door closed, but it's "tight" as well - by hand, cannot detect any looseness in the door - can rock the van all around from grabbing, and moving the door, while closed.
Also, when the door is open, the TOP roller has a lot of play in it...how hard is it to replace this, and could it be the cause?

The top roller really doesn't support much. The bottom bracket has two plastic rollers on it. One vertical which supports most of the door weight and a horizontal one which guides the door just like the top one. The vertical one is the one that usually wears out (it really doesn't wear out, the plastic wears down on the roller and it gets smaller in diameter). I repaired my door with a 3/4" (I think that is the correct size, tho I might be wrong) plastic cover, the kind that is slipped over the ends of tubular steel furniture. Disconnect the bracket from the door and separate the two pieces. Then you can jiggle the vertical part out and slip the plastic dodad over it. Reassembly is just as easy, and the adjustment can be seen by the old impressions on the different bracket parts.
