Yes, One hole. Used a rubber gasket cut out of an old inner tube so it wouldn't scratch the hood. Also had to use a longer mounting bolt as the one that came with it was a little too short.
I do service work at the Mack plant Medical Dept. in Hagerstown, MD. Nurse got me a catalog so I could buy my bulldog.
