Roush reprograms the pcm for you, if you purchase our kit there is a prelabeled FedEx box in which you will mail the processor to us and we will flash it for you. No programmer or chip is needed.

The driveshaft loop is basically a loop that goes around the driveshaft to protect the driveshaft from falling out. That way if you have failure you wont catapult the vehicle. Balancing the driveshaft will make sure the driveshaft does not vibrate at higher speeds. The 4x2 are notorious for having a week driveshaft from what I understand.
The flash, puts all the required programming into your pcm so that the computer will adjust for the supercharger. That way your vehicles parameters will be adjusted for Air/Fuel Ratios, shift points, etc, etc.

The modern day car computers (ECM, ECU, EEC's) contain flash chips, whereas instead of attaching a chip/dongle to the back of the computer, or replacing the memory chip (eeprom), you can "flash" the new code/data into the computer using a cable and a laptop, replacing the existing, factory set code/data.
Personally, I wouldn't send anyone your factory orginal EEC. Instead, I'd buy the same EEC from an auto parts store, rockauto.com, or on ebay (usually the cheapest), then send THAT one, this way should you ever decide to remove and sell the supercharger kit, you can simply remove it, plop the original EEC back into your truck, and turn the key for happy driving.
Some companies offer the option to retain the factory program and install a switch that allows you to select between factory settings, and the new supercharger settings. Not sure if Rousche or Trilogy offer that but if they do, that's another option of course.
Me, I'd still send "whomever" a spare EEC just to be safe.
And if you're really worried about driveshaft twistage, upgrade to an F350 driveshaft, and spend the time to figure out the custom u-joints. You won't break it.
There are a number of programs you can use - the only one that I am aware that is completely customisable is SCT software.
Supercharger "kits" will include a tune program. Some will use existing tuners/chips (like I believe Trilogy uses the SCT), others will have thier own chip (Kenne Bell for example).
I thought I would add to this - all of the programmers are capable of doing this - you just need to give them your settings, and they will write a program for you, however the SCT is the most adjustable by someone that has the "Pro-racer" package.

Balancing a driveshaft essentially just makes the weight on the shaft on the more even to reduce vibration - and prevent the above happening.
A 2 piece shaft (ex F350 - like frederic said) could always be an investment.

I couldn't agree more...a loop is cheap insurance if I do say so, and some track officials wont let you run if you have slicks and no loop.
