Heater Control Valve question

This is the second one I installed. I returned the first one telling the parts guy it was leaking. Now the second valve is doing it again. Perhaps I have one the the hoses incorrectly installed. I cannot think of anything else that could be my fault. Four hoses and a vacuum line. Seems pretty simple.
If I remeber correctly (I am at work and can't look at the valve) from the front part of the valve, one hose goes to the thermostat housing and the other goes to the lower rad.
How do these hoses connect to the valve on your truck?
e.g. thermostat to upper valve port, lower rad to lower valve port.

1. The hose coming off the top front of the unit (you can just barely see the end of it in the pic) goes down to the front of the motor and seemingly connects up to water pump (or very close to it). It is by far the longest hose.
2. The hose coming off the bottom front of the unit (can't see it in the pic) connects up with the T-stat housing.
3. The hose coming off the top rear of the unit connects up with the passenger side heater core connection on the firewall (you can see it start turning to the left in the pic).
4. The hose coming off the bottom rear of the unit connects up with the driver's side heater core connection on the firewall.
