For the lower mount just use the stock mount on the axle, its just a 3/4" bolt.
For the upper mount on the frame you will need to fabricate something. A guy I work with did a pretty good job on his upper mount when he did an axle conversion on his F150. I'll see if I can get a picture to give you an idea of what you will need to do.
