That depends on the kit. Some require the use of Mallory metal in the crank to add weight. A trip to a good machine shop that can do balancing is in order, it's not really something that you can do yourself without specialized equipment.

No, they'll need the rotating assembly since they hav to static balance individual components first, and then dynamic balance the actual rotating components. Take them the harmonic balancer, clutch and flywheel (or flexplate...I don't think they need the converter but you can ask, if have A/T).
Also take them the rings and wrist pins. The pistons should not be on the rods yet as these will all have to be weighed and balanced seperately.
