They recommend part number ZX786952TF, which is Extension Bolts for the dually adapter.
If your truck is lowered too much, your probably going to have to do some cutting to the wheel arch.
Nothing wrong with doing it but it is a visual package only, it is not a good way to increase your payload regardless of what anyone claims. If you use it as a method of increasing payload you will simply wear out your wheel bearings and prehaps your axles as well. I run mine with 32 lbs air pressure on the inside wheels and 20lbs on the out side in an attempt to minimize the pressure exerted on the bearings. Real dually's have 2 sets of wheel bearings per side
JC Witney will sell the adaptor, extended lugs, extended air fillers, and or the fenders as a pack or seperately.
I was thinking that down the road I will build a 5 liter Ranger conversion and install the fenders to cover a nice set of 50's on the back.
Mine has got a load of parts on it including a wicked set of custom build running boards. They are built out of aluminum checker plate and form a vert tight fitting wedge from the front fender to the extended rear fenders. Has a Lund visor, wicked chrome tube bed rails, tonneau cover, billet grill. The truck is in real nice shape inside and out, paint is decent and it looks real good.

As for the gear ration. No reason to change it but when you add that much mass to the drive wheels it will eat up some horsepower. Probably a noticeable amount. Its the same principle that eats horsepower when you switch to larger diameter tires and rims. It can add up to a surprising reduction. I remember an issue of Two Guys Garage on TV and they lost, I beleive it was 7 or 8 hp at the wheels just by going up a couple of inches in rim size. They happen to have a dyno so they were aware of it, most people don't seem to realize.
