03 Overheat?
If there is a cooling system problem under load, it would lead me to believe the EGR cooler is failing. During these incidences, is coolant spewed severly out of the degas bottle cap? The reduced power is from defueling/derate due to high ECT.
There really isn't that many potential things that can cause this issue. The head gaskets could be faulty, the EGR cooler could have failed...other than that, only a bad water pump or a stuck thermostat will cause severe overheating, and if it were one of those two the truck would exhibit symptoms even with no load.
If it was my truck, number 1 priority would be to ensure the EGR cooler wasn't busted/melted. Second would be headgaskets- let the dealer know that a compression leakdown test isn't worth a damn if this is the case, because they will only fail under high load and reseal once the engine is cooled, since they are multi-layer steel gaskets. Make sure they check for exhaust gasses in the water jacket and cooling system-if they are present, it is most certainly either the cooler or the gaskets. Generally it is acceptable by Ford to replace both at the same time if exhaust is present in the coolant.
Is the cooling fan engaging when this happens? You'll know it if it does, the fan is extremely loud. If not, the vistronic fan clutch and wiring may be an issue to be looked at as well.
