Most likely you will need a P/S box, P/S pump and brackets and all the lines. I'm not sure if the tie rods and all the steering components are the same for a non P/S equipped vehicle though. The easiest way is to find a donor truck to get all the parts from.

Does the PS shaft have to be from a PS 3 speed column or can it be from a PS auto or a 4spd PS ?

it can be from any of them. the shaft is the same as long as it is power steering.
