engine/heater running cold

After 4 hrs of highway driving, my '93 E350 was still registering cold on the temperature gauge and when I turned on the heater with the temperature set at high, cold air was blowing from the fan. Any ideas?

Tstat is always the first thing to replace. It is the only thing that regulates your engine temp.
