In older cars, the PCV was unmetered air that was assumed to be a certain amount under different conditions. Today, the breather is metered air so unless you have valve cover (or other) gaskets leaking or the filler cap/dipstick off, it shouldn't affect the mixture.
Some causes include a weak or bad pump, a partially plugged fuel filter, a bad fuel pressure regulator, plugged or dirty injectors, bad IAT sensor. There are others, but this just gives an idea there isn't a short answer.
Positive Crankcase Ventilation. It sucks the impurities out of the crankcase and reburns them in the engine. You might try cleaning the MAF. If dirty, it will not properly read incoming airflow to the engine.
I would also replace the PCV valve with a new Motorcraft unit.
Also, look at the entire hose assembly from the PCV, it will often crack in multiple places due to dryrot. It is a known defect that Ford is happy to sell you a $65.00 assembly to fix , can't just order the boots or anything like that. I got the same codes and chased it with everything from PCV valve all the way to fuel filter, turned out there was a very small crack by the "Y" piece at the side inlet to the manifold, yes, the hose goes into the manifold in 2 places. Took about 15 minutes and the above hose assembly, (watch out there are 4 different ones, order it with your VIN in hand), reset the computer, and ON DOWN THE ROAD!
